H4U (Health for You) is the Thai government's official application, developed by the Ministry of Public Health, Thailand, used to display personal health record or health status of owner including health certificates related to COVID-19. The app is developed and released in full compliance with the protection of the user's personal data and with current legislation.

This app is part of a response to COVID-19 and accesses only information collected to support COVID-19 efforts and epidemiological research on vaccination states, laboratory results (ATK/RT-PCR), infection history, and health certificate to enable people who own health information to participate in various activities.

H4U does not and cannot perform diagnostics. It makes recommendations about what to do next based on the user's history of exposure to potentially contagious users. However, the app is not a medical device and should not be used in place of a doctor.

The use of this app is entirely voluntary, and it does not necessitate access to Thailand's COVID-19-related services.

Collected data.

To help the Thai Ministry of Public Health understand the health situation and disease control including the current vaccination situation, vaccine effectiveness and to track the vaccine adverse effect under the Department of Medical Sciences' conditional approval for laboratory conditions, the Thai FDA's Emergency Use of Medicinal Products AD 2012, and the Department of Disease Control Communicable Disease Act AD 2015.

The app gathers:

1. Your demographic data, The citizen ID is especially important for the government to correctly match and display vaccination records, laboratory results, infection history, and health certificates from the national database. To correctly identify the health certificate, the following information was gathered: name, last name, gender, prefix, and phone number. When you have access to healthcare services by using this app, phone numbers are collected for password restoration and to allow our healthcare personnel to reach out for vaccination adverse event follow-up purposes only.

2. Address (Province, District, Subdistrict code) were collected to select for organizing health services and making appointments for health services.

3. The ID behind your citizen ID card was entered only to verify your identity with the Ministry of Interior database. We did not collect either of those data and discarded it at the end of the process. This step is especially important to prevent identity fraud.

4. The COVID-19 Digital Health Pass verifier function uses the front camera to scan the QR code on the pass issued by the Thai Government and allows the user to verify the status of the health pass. The verifier function only serves as a verifier and does not collect any data.

5. Your location, phone contacts, or health sensors were not collected by the application.

6. The information stored on the smartphone is encrypted.

7. The app's and the server's connections are encrypted.

8. All data, whether on the device or on the server, is deleted when it is no longer needed.

All collected data will be similar to those collected at vaccination site registration desks, laboratory testing sites (ATK/RT PCR), and hospital without application.

Sharing data.

1.The collected were shared with the health care personnel for public health purposes. The data will not be shared for commercial purposes without your permission.

2. Information gathered to help the Ministry of Public Health manage Thailand's health situation and develop health promotion plans.

Other information.

1. The use of personal data is governed by the Personal Data Protection Act, B.E. 2562 (2019) of the Kingdom of Thailand.

2. The Ministry of Public Health is the body in charge of collecting data and deciding how to use it.

3. The data is stored on servers in Thailand and managed by government bodies, with the sole purpose of containing the COVID-19 epidemic or conducting scientific research.
